How It Works:MBA (Medical Bill Assistance)

  • Upon reviewing Criteria, an uninsured breast cancer patient should download an application, or call 303-452-1898 to have one sent via mail to their home address.
  • All applications must be completely filled out to be considered. Don't forget to sign and send the Patient Authorization and Criteria signature sheets as well.
  • The Mickaela Foundation Executive Board of Directors then reviews and verifies all applications for correct, complete information.
  • A generalized profile is created from the information, then submitted to the Executive Board of Directors at the next scheduled monthly meeting. Personal information submitted on applications is strictly confidential and not shared with any other entity or company.
  • Upon approval, listed medical bills are requested by fax or mail. Legible copies are fine. The facility will be contacted for current amounts owed, then the statements are submitted to the Treasurer for payment within 30 days of receiving the bills after approval.
  • Confirmation will be sent within 30 days for payment.
  • Applicants may apply as many consecutive months as needed, however, funding is not guaranteed. If an approved applicant does not submit bills within the month they have been approved for, they will need to reapply for the next month.
